If he is still alive and telling this story to a doctor then including the doctor from the beginning would mitigate a lot of the squeamishness over suicide. I actually think its a pretty cool idea but I have no problem accepting that suicide leads directly to hell in your book's world.

I do plan on adding the doctor. He plays enough of a role that I think the omission might put off an agent more than the issue of suicide, because it might come off as a bait and switch.
Now to finish the derned thing. My biggest concern now is how to make the character stop appearing like such a wuss at times, and keep a reader's interest through the parts of the story that do NOT pertain to hell.
