I have read on various agent blogs that it doesn't matter that much which formula you go with...however, most say to just use the count from your word processing program. I don't think that Nathan put the friend/foe lists on the forum. I think they just come as part of the forum automatically. :) Kind of silly. Internal monologue in 3rd person POV needs to be italicized or in quotations marks. Either are correct, so you can mix and match between those all you want. My personal preference is italicized.

I believe I see 'Sam said' far more than the other, but I can't be positive because good dialogue tags go unnoticed by the reader. For some reason, I don't really care for 'said Sam.'

There isn't really a rule for this. Both are grammatically correct. It is just author preference.
