You would copy the 5 pages and paste them into the body of your email - but you'd only do that if the agent's query submissions required it :)

It is a description of your novel - not to be confused with the "Back of the book" blurb. It can be a 1 sentence blip, a 1 paragraph, 1 page or 10 page description of your book. Basically, it tells someone what your book is about, giving more than a query letter would.

My first impressions of your query are that it doesn't appear to be a YA novel. If you want it to be young adult, the main characters need to be under 18, and having Liza's crush be a 40s-something psychologist seems a little stalker/scary-ish. I'm no expert, and I haven't read all you posted, but IMO, "Five People You Meet in Heaven" by Mitch Albom and "The Shack" by William Paul Young would be Relgious Fiction. I don't know about the others...
